The 5-Minute Feedback Form for the KVP Learning Cycle
Many WHP coordinators do not know whether a measure is effective after it has been implemented — because they never asked. The effort required for meaningful feedback is low, and the benefit for the annual report is significant.
Why are three questions enough?
The GKV Prevention Guideline requires securing and improving the planning, process, and outcome quality of measures. For a single measure, this can be broken down into three dimensions: process quality (Was the measure well organized?), resource development (Did the measure provide real benefit?), and acceptance (Would participants recommend it?).
Exactly these three dimensions can be assessed with three questions — if they are formulated correctly.
The copy-ready questionnaire
This questionnaire can be used as a paper form, Google Form, or digital survey tool. Rating on a 1-to-5 scale (stars or numbers):
**Question 1 — Process quality:** "The measure was organized in such a way that I could easily integrate it into my workday."
**Question 2 — Resource development:** "I learned concrete methods or exercises to strengthen my health independently."
**Question 3 — Acceptance:** "Would you recommend participating in this program to your colleagues?"
How do you evaluate the answers in the annual report?
The averages across all three questions result in the "measure score." For the GKV report: compare before and after (if the same measure is repeated) or benchmark between different measures. A score below 3 on question 2 is a clear signal: the measure is not having a lasting effect — reconsider next year. How a results report for management can be structured is shown in the guide WHP results report for the board.
How do you put the measure in place?
- 1Prepare the questionnaire as a digital form or paper slip
- 2Enter three questions (process quality, resource building, willingness to recommend)
- 3Set up a 1–5 star rating as the scale
- 4Distribute the questionnaire at the end of each measure (not mandatory to be anonymous, but recommended)
- 5Aggregate the results: calculate the average per question and measure
- 6Document in the health committee (AK Gesundheit) minutes
- 7For the annual report: summarize measure scores in a table
