Apply for GKV Funding for WHP
What does health insurance pay for workplace health promotion (WHP) — and which measures actually qualify for §20b funding?
Note: this covers German statutory health insurance law. GKV funds WHP under § 20b SGB V across three action areas — work design, healthy lifestyle, and inter-company networking — requiring a coordinated process, a documented steering group, a needs analysis, and employer co-investment.
GKV-Fundable WHP Action Areas Under § 20b SGB V (GKV Prevention Guide 2025)
| Action Area | Fundable Measures (Examples) | Excluded |
|---|---|---|
| Consulting on health-promoting work design | Workplace walkthroughs, risk assessment consulting, ergonomics workshops, health-oriented leadership training, concepts for activity-friendly environments, healthy cafeteria programs | Technical equipment (chairs, desks), personal protective equipment, construction work |
| Health-promoting work and lifestyle habits | Stress management seminars, back-health courses, nutrition workshops, smoking cessation programs, exercise programs, relaxation techniques — each derived from the needs analysis and run by qualified providers | General workplace sports without guideline compliance, fitness equipment, standalone offerings not embedded in the WHP process |
| Inter-company networking and consulting | Participation in WHP networks, inter-company information events, multiplier training, consulting through WHP coordination offices | Political party activities, promotional offers for products/facilities, research projects without an intervention link |
Which WHP measures does health insurance actually pay for?
Many companies assume health insurance simply reimburses sports courses. That's wrong — and it leaves possible funding unused. § 20b SGB V (the German Social Code governing statutory health insurance) doesn't fund isolated individual measures, but a documented health promotion process.
The GKV Prevention Guide (version dated December 17, 2025) defines three action areas for workplace health promotion. First: consulting on health-promoting work design — meaning condition-based measures that shape working conditions, leadership, and company structures in a health-promoting way. Second: health-promoting work and lifestyle habits — behavior-based measures such as stress management, physical activity promotion, nutrition, and addiction prevention. Third: inter-company networking and consulting.
What matters is embedding: a standalone yoga class booking or a fruit bowl in the office is not a GKV-fundable WHP measure. Only measures derived from a traceable needs analysis, approved by a steering group, and followed up with regular evaluation qualify for funding.
What does the GKV require from providers and employers?
For behavior-based measures in the workplace (e.g., stress management, back-health courses), the quality criteria of the GKV Prevention Guide apply: among other things, the fund checks the provider's concept and qualifications. Certification by the Central Prevention Certification Body (ZPP) exists only for individual prevention courses under § 20 SGB V; no certification is provided for workplace health promotion (WHP) under § 20b SGB V.
For the employer itself, the GKV Prevention Guide requires: a written company policy or letter of intent for WHP, employee/works council involvement throughout the process, measures based on a needs analysis, an internal steering group (a health working group, ideally involving the occupational safety committee under § 11 ASiG — the German Act on Occupational Physicians, Safety Engineers and Other Occupational Safety Specialists), regular evaluation and documentation, and demonstrable co-investment by the company — financial and/or in staff time.
Important: all services must be coordinated with the funding health insurance fund in advance. Under the GKV Prevention Guide, billing for services that weren't pre-approved is explicitly not permitted. Book first, seek funding later, and you get nothing.
- Written letter of intent / policy in place
- Steering group (health working group) active and minuted
- Needs analysis conducted and documented
- Measures derived from the analysis — not chosen arbitrarily
- Qualified providers for behavior-based measures, agreed with the fund (ZPP certificates exist only for courses under § 20)
- Pre-approval with the health insurance fund completed
How do § 3 No. 34 EStG, § 20b SGB V and in-kind benefits fit together?
Health insurer funding, the tax allowance, and in-kind benefits are separate routes that can exist side by side. None of them automatically refinances WHP: the insurer funds projects by agreement, while the tax allowance makes employer benefits tax-free. Germany's Federal Ministry of Finance has published its own implementation guide on the tax exemption.
Tax allowance — § 3 No. 34 EStG (Income Tax Act): Up to €600 per employee per year is tax-free for employer-funded health promotion benefits that meet the requirements of §§ 20 and 20b SGB V. This can cover certified prevention courses under § 20 SGB V and — without certification — measures within a structured workplace health promotion process. In practice: if the employer pays for a course, for example, on top of the wages already owed, no payroll tax or social security contributions are due on it up to this limit. The €600 is a tax allowance — an upper limit, not a target and not a payout; in practice a fraction is often enough. For 50 employees, the limit works out at €30,000 per year — a ceiling, not a budget that has to be used up. Only your tax advisor can confirm the tax exemption.
Health insurer funding — § 20b SGB V: the health insurance fund funds guideline-compliant WHP projects — as a sum for a single measure, a health day for a few thousand euros, or an annual budget (e.g. €5,000), not as an amount per head. Each fund agrees the amount and form itself; the company has no entitlement. Prior agreement is mandatory.
Important: if the health insurance fund has already subsidized a course fee, an employer subsidy is tax-free only for the amount exceeding it. Insurer funding and the tax allowance are therefore not mutually exclusive — but the same euro only counts once. Coordinated, complementary measures can draw on both.
In-kind benefit — § 8 (2) sentence 11 EStG: in-kind benefits of up to €50 per month remain tax-free (a monthly exemption limit that cannot be carried over to other months), e.g., for corporate fitness memberships. This route can be used independently of § 3 No. 34 EStG — it is a separate route of its own.
How does the application for health-insurance funding work?
For WHP funding under § 20b SGB V, the company contacts a health insurance fund that covers at least some of its employees — not necessarily the largest one. The first point of contact is the fund's corporate client advisory team or the regional WHP coordination offices (www.bgf-koordinierungsstelle.de).
The process runs in five steps: first, initial contact with the fund and an initial consultation. Second, a joint preparation phase — the fund often actively supports setting up the steering group and needs analysis. Third, a written funding agreement is concluded that defines the measures, scope, and funding shares. Fourth, implementation of the measures with documentation. Fifth, evaluation and reporting as a prerequisite for follow-up measures.
Timeline: experience shows that several months pass between the initial inquiry and the first funding payout — plan for the lead time. If you want funding for the current year by Q4, you need to start by Q1 at the latest.
Why does funding fail the health insurer's review?
Mistake 1 — No prior agreement: a common reason funding applications get rejected is that measures were booked without a prior agreement with the fund. The GKV rule is unambiguous: services not pre-approved will not be reimbursed.
Mistake 2 — Isolated one-off measures: a mindfulness course that HR books once, without embedding it in a WHP process, does not qualify for funding. The fund checks whether an analysis, a steering-group decision, and an evaluation plan are in place.
Mistake 3 — Unsuitable providers: for behavior-based measures, the fund checks the provider's concept and qualifications against the guide's criteria. Clarify this with the fund before booking. A ZPP certificate is not a requirement here — it only exists for courses under § 20 SGB V.
Mistake 4 — No demonstrated co-investment: the GKV Prevention Guide requires financial and/or staff-time co-investment from the company. A business that tries to push 100% of the costs onto the fund without contributing its own resources gets no funding.
Mistake 5 — Missing documentation: no steering-group minutes, no written needs analysis, no measures plan — these are typical formal grounds for rejection.

What's your company's workplace-health maturity?
Your maturity level shows how far your measures already sit within a structured health promotion process. Under German tax law, such a process is one way for measures to be tax-free up to the allowance of €600 per employee per year (§ 3 No. 34 EStG) — only your tax advisor can confirm this. Answer in about 7 minutes, no sign-up.

Key takeaways
- § 20b SGB V funds processes, not individual measures — no steering group and needs analysis, no funding.
- All measures must be pre-approved with the health insurance fund — retroactive reimbursement is not possible.
- For behavior-based measures, clarify the concept and provider qualifications with the fund in advance — a ZPP certificate is not provided for under § 20b.
- Separate routes: § 20b health insurer funding is a project sum with no entitlement; separately, § 3 No. 34 EStG is a tax allowance of up to €600/employee/year, and the €50 in-kind benefit is another route of its own.
- WHP coordination offices offer free initial consultations for companies of any size.
Frequently asked questions
Is there a minimum company size for § 20b funding?+
No — even small businesses can apply for § 20b funding. For anonymized GKV health reports, however, each fund sets its own privacy-related minimum number of insured employees — below that, detailed reports aren't possible. The WHP process itself is open to companies of any size.
Which health insurance fund should I contact?+
In principle, any fund with insured employees on your staff. In practice, it's best to start with the fund covering the largest share of your workforce — it simplifies evaluations and data reports. Alternatively, WHP coordination offices (www.bgf-koordinierungsstelle.de) offer cross-fund initial consultations.
Can I use § 3 No. 34 EStG without applying for GKV funding?+
Yes. § 3 No. 34 EStG is a tax-law allowance that the employer can use independently of GKV — as long as the measures meet the quality criteria of §§ 20, 20b SGB V. A ZPP-certified course is generally sufficient proof; for behavior-related measures with no certification option, what counts is the structured workplace health promotion process (needs analysis, employee involvement). Only your tax advisor can confirm the tax exemption. GKV funding is a separate mechanism.
What happens if we don't have time for steering-group meetings?+
Then there's no GKV funding. The steering group isn't an optional extra — it's the central checkpoint of every funding agreement. As a rule of thumb from practice, four well-documented meetings a year (60–90 minutes each) are often enough. The effort is manageable.
Is workplace sports funded under § 20b?+
Only guideline-compliant workplace sports. General sports offerings that aren't embedded in the WHP process are excluded. Exercise programs derived from the needs analysis and led by qualified instructors can qualify for funding — clarify this with the fund in advance.
Can the employer pay for a gym membership tax-free?+
Not under the tax allowance of § 3 Nr. 34 EStG: gym and sports club fees and corporate fitness contracts are not eligible there. A benefit in kind under § 8(2) sentence 11 EStG is possible — tax-free up to €50 per month, as a separate exemption limit alongside the allowance. German statutory health insurers do not fund gym fees under § 20b SGB V. Your tax adviser confirms whether it fits in the individual case (German law).
